Why Most Back Pain Treatment Fails
The dirty secret of back pain treatment is that most of it doesn't address the actual cause. Generic exercise programs, passive treatments like heat packs, and advice to "take it easy" can actually delay recovery and lead to chronification — where acute pain becomes chronic.
Research consistently shows that: early active movement outperforms bed rest, manual therapy combined with exercise is more effective than exercise alone, individualized treatment beats cookie-cutter protocols, and addressing movement patterns prevents recurrence.
What's Actually Causing Your Back Pain
Not all back pain is the same. Common causes I see in my South Florida practice include: lumbar disc issues (herniation, bulge, DDD), SI joint dysfunction, facet joint irritation, hip and glute weakness causing compensatory lumbar loading, thoracic stiffness driving lumbar overload, and poor core coordination (not just "weak abs").
Before treatment begins, I conduct a comprehensive movement assessment to identify your specific driver. This changes everything about how we treat it.
Phase 1: Pain Resolution (Weeks 1–3)
The goal in the first phase isn't to get you to the gym — it's to calm the system down and restore basic movement. This involves manual therapy (joint mobilization and soft tissue work), neural tension reduction if the sciatic nerve is involved, breathing and core activation re-education, and guided movement within pain-free range.
Most patients experience significant symptom reduction within 2–3 sessions when treatment is appropriately targeted.
Phase 2: Functional Restoration (Weeks 3–8)
Once the acute phase resolves, the real work begins. This is where most insurance PT stops — but it's where recurrence is prevented. Phase 2 covers hip hinge mechanics and deadlift pattern rehabilitation, hip strengthening (glutes, external rotators), thoracic mobility work, and progressive loading to restore full capacity.
Phase 3: Return to Performance
Whether your goal is getting back on the golf course, returning to CrossFit, or simply picking up your grandchildren without fear — Phase 3 gets you there. This phase is highly individualized and focuses on sport-specific or activity-specific movement patterns, progressive strength training, and building confidence in your back's capacity.
