Pricing FAQ

Have a question not answered here? Call us or book a free consultation.

How exactly does the billing work?
At the start of each month, you're billed for the sessions you had the prior month. There's no package to pre-purchase and no sessions that expire. You book what you need, we track it, and you're charged on the 1st. Cancel anytime with 30 days notice.
Why doesn't Empower Fitness take insurance?
Insurance contracts dictate session lengths, visit limits, and billing codes — not your clinical needs. Operating outside of insurance lets Dr. Ezra give you 60 uninterrupted minutes, see you as often as your goals require, and make clinical decisions based on what's actually best for you. Most clients find the true cost comparable once they factor in co-pays, deductibles, and lost time driving to a clinic.
Can my insurance reimburse me for Empower Fitness sessions?
Potentially, yes. We provide detailed superbills (itemized receipts with CPT codes) that you can submit to your insurance for out-of-network reimbursement. Reimbursement varies widely — some clients recover 40–70% of costs, others receive nothing. Call your insurer before your first session to ask about out-of-network PT benefits.
Is there a cancellation fee for individual sessions?
We ask for 24 hours notice to cancel or reschedule a session. Late cancellations (within 24 hours) may be charged 50% of the session rate. Life happens — we apply this policy with common sense, not rigidity.
I'm not sure how many sessions I'll need — how do I figure that out?
Start with the free 15-minute consultation. Dr. Ezra will listen to your situation, assess your goals, and give you an honest recommendation on session frequency and type — with zero pressure. Most clients dial in their rhythm within the first 2–3 weeks.
What's the difference between a Physical Therapy session and a Dr. Ezra session?
Physical Therapy sessions can be led by any licensed DPT on our team under Dr. Ezra's clinical direction. Dr. Ezra sessions are 1-on-1 with Dr. Ezra Miller directly — best for complex cases, performance optimization, longevity programming, or when you want the clinical director's eyes on your specific situation.
